2-Why have you decided a Miniature Bull Terrier is the dog for you?

3-For what purpose(s) do you want to purchase a Mini Bull Terrier? (Family Pet, Show, Obedience, Agility, Breeding)

4-Do all family members agree to the purchase of this puppy?

5-Do you currently have any dogs or other animals in your home?

21-What type of personality are you looking for in your puppy?

23-If there ever came a time when you could no longer keep your Bull Terrier, describe what you would do with your dog? 

24-In most cases, we will not ship a puppy. Is personal pick up a problem?

25-Are you willing to be on a waiting list for a year or more? 

26-Are you currently on any other Bull Terrier breeders’ waiting lists? 

27-Any puppies sold as a Pet will be sold on an “AKC Limited Registration” with spay/neuter agreements. Do you have any problem with this? 

28-Are you or would you ever be interested in Showing? 

29-Have you ever bought from, or given-up a dog to, a shelter or pound? If so why? 

30-Do you have a regular Veterinarian? (If yes list, name, address, and phone#)

31-If you are chosen for our waiting list would you be able to give us name/address/phone numbers of a vet cardiologist and a vet ophthalmologist near you? 

32-Would there be any problem with us or a member of a local BT club coming to your home with a Bull Terrier to visit and do a home check? 

33-Do you have a way to send updated pictures and information on the progress of a puppy over the internet? (Digital camera or picture phone, email or web site)

34-Are you willing to be in regular contact with Evolution Mini Bulls, if chosen for a puppy?
